Create a Mobile responsive website that is questionnaire/form driven.

There is the need to create an new smart and effective data driven web based solution for an existing manual and time intensive system questionnaire/Forms driven system. This is how the current system works... 1. Client calls up 2. Appointments are made and fees are paid. 3. Jobs are allocated to various staff members to handle certain geographic locations and all those jobs are organised for them accordingly. 4. Onsite, the staff go through the questionnaire/form, fill in the details, take some pictures 5. Come to office, type in the contents of the form into a computer 6. Submit that filled-in form to their supervisor for review 7. The supervisor then releases that form as a "report" to the client with additional documentation (standard). Job is complete & the data is locked and stored for future reference, never to be altered again. This whole process can be managed using iPads/iPhone/android devices by creating mobile responsive websites. This is how the end product would look like Booking 1. Client goes to the website, looks up the booking calendar (anonymised) and finds a date/time that is suitable for them. 2. Client fills in the booking details and submits the form. 3. Client is taken to the Payment system (PayPal or something...) that accepts the payment and the booking is locked in. At this stage, the supervisor is *notified* of the job booking with the relevant date/time info. The Job 4. The Supervisor can assign specific jobs to respective employees or can select an employee and the system can (based on their geographic preference) suggest upcoming jobs relevant to them. 5. Once the job is assigned to the employee, the employee is *notified* with the relevant details of the job. 6. The employee then can select the job, and the address details are present, clicking them will use the built-in maps to help navigate the employee to their destination. 7. Onsite, the employee opens the job, and starts to fill in the details. The questionnaire are simplistic in nature with only parent-child interdependency built into them e.g. if on form 1, the employee selects q.10 option 2, it will then allow the employee to see and select option on form 2's q3. 8. Onsite, the employee will encounter a question on the form that says, please attach pictures to support your statement. they can then use the built-in smartphone's camera and click pictures, that will automatically re-size and present themselves while the unaltered copy will be attached to the form. 9. Onsite, the employee can go back and revisit/change their answers multiple times, until they choose to send it for review and submit the form, after which the job's gone. In the office. 10. The supervisor, review the submitted jobs, error checking any spelling mistakes and/or concerns. 11. The supervisor, can send that job back to the employee for redoing it again too. 12. Once its deemed complete, the supervisor will print and publish the report. This will print some standard PDF's + the questionnaire that was filled by the staffer + any pics + disclaimer all into one PDF and email it to the Client and/or ask the client to download it form the website with a special link created for them. This action also locks the record preventing future edits (for legal reasons) Reports 1. Jobs todo per locale/employee 2. Supervisor - staff report 3. Simple GL day/month/year/date range We have the forms in Word as docx files. We'd like for this solution to be hosted on a scalable web platform too if possible, so if needs be we can add extra power/security/features all on-demand, eg. heroku, etc... suggestions welcome... Finally, the winner should be able to demonstrate with some proof of prior work, quality of product & creativity will determine the winner. There is ongoing work here after this project completes, it will all depend on this first phase. Good Luck =)
