Create a Video - Time is Muscle Series "Size Matters"...

I am looking for an Intro Video for an upcoming series I am filming called " Size Matters". I want the Intro to look similar to an Intro I have see on a Youtube Channel [login to view URL] (Time - 3:47).

I have attached some Transparent images I want featured in the Video. In addition I have attached an instrumental to play as video is playing. The opening Title Should flash the logo's and then display Large revolving font saying "Size Matters".
