C# small customization (printing)

I have small C# application that currently writes some data to remote MySQL. Now I would like to write another section (tab or anything) - purpose will be to print 3 texts to predefined positions on page - I have some preprinted form (with empty textboxes) that is in printer and this application should print three texts into those textboxes. I will provide form in pdf to the winner. It's not public. So your task is to create some tab in an application or some section divided somehow with: Enter code: ____-____-____-____ [Print] There will be textbox for entering code (in format dddd-dddd-dddd-dddd) - it would be nice to have masked textbox. And than button "Print". After clicking print, application will connect to mysql and select one row that contains those three texts we need to print. After selecting them (or possibly displaying some messagebox that such code was not found in database) application will print result. The print can be using some rdlc report or what and can be displayed and it can require user interaction (clicking another "Print") or what - it's up to you if it will be fully without preview (would be better) or what... If I am satisfied with results or you spend more time on it and it's really nice I will give you more development of this tool and also the next bigger tool I need. So... try to do your best ;-) Btw connecting to db is already written so you don't need to solve this. Just copy or refactor it somehow to be universal.
