Database generator for practice problems

I need a simple database or input program that will substitute user defined values into the a, b, c, d, m an n values in the basic properties of algebra (see attachment). The substituted values will be numbers, text and or object (other forumulas). I've attached an example of what an output should look like for individual forumlas. The final output will be a word file or text file that can be copied and pasted to a document. erje

The attached zip will has the properties, examples of the output should look like, and a concept document to give perspective on how the project fits in to something larger.
