Develop Simple Facebook App

Looking for someone to develop a simple Facebook app with an integrated fangate. The app is simply a 2-question quiz that awards a certificate (automatically filled in with the user name and profile picture) based on the answer. This is what the app must do: 1. Ask the user to like the page before he can use the app; 2. Grab user name and email and forward to my autoresponder; 3. Ask 2 multiple choice questions, and grade the user based on his answer; 4. Grab user name and profile picture to fill in fields on the appropriate certificate; 5. Post completed certificate to user's wall and optionally, app fanpage; 6. Optional: send automatic email to user with certificate download link I want to get working on this asap, so please do not bid if you are not available to start at once.
