create php forms and mysql database

I want to create two php forms and a mysql database. Both are defined in the attached rtf documents. You can work on your own and then export the database and post the files here. I want to be able to put the forms in my existing website that I am building now.
I am not a database guy and I welcome any input and suggestions you may have for better organization and structure.
This is the first of several forms and interactions on the site, so please only bid if you are interested in ongoing work. I will add bids for that as we progress.
I want validation for phone number and email fields and captcha. They also must be designed so that they cannot be hacked or used for spam. Also keep in mind to keep the code clean and commented for future use.
I also want 1 page for the [url removed, login to view] login and view their data, 1 page for teh subscriber to login and view their data and 1 page for an admin to login and choose to view all subscribers or all consumers.
(note the file upload feature for images and logos)
