Problem: We have 5-8 page PDF documents with medical information that we want to use to prepopulate a different PDF document. some of the data will need to be modified automaticaly on the new form (Macros or formulas).
Requirements
1. Will likely require creating a database that can then be used to populate the new database form.
2. Some of the info needs to be modiefied automaticaly prior to being put in new forms.
3. Must speak english as will need instuctions on how to create forms
4. Must be high quality and offer support for 9 months
