Create Simple online or desktop application

Hi, I have created 2 videos to explain what I want. [login to view URL] What I want is a simple "fill in the blanks" online or desktop tool. The video shows you exactly what I want. Basicly, I would be able to input my own text in a box, there will be about 8 boxes with different headings. After I have inputted into each box what i want I will have the option at the bottom of the form or program to save what i just inputted as an html file(i should be able to name it) or as an text file. On the application I would need to have the ability to link to a video/image/document and input an url where the particular file is located. The video will show you where this is needed. I will need all rights to this since I may sell it in the future. I am not sure if a desktop or online version would be easier. You can tell me what you think and justify your logic. 1. Please submit a proposal and what you will be creating for me, to rule out any misunderstanding. 2. Please be detailed oriented since only those applications would be considered. 3.
