Booking system / Membership System

Simple database driven records management and jail systems booking.... this should be a php script, simple, easy to skin. this sides on law enforcement booking system. however is a very simplified version. ## Deliverables FIRST - I apologize for spelling and grammar IF its an issue. This project should be simple. I need a simple, database run, mysql Bookings system. Such as the police departments would use. Please note this script should be easy to skin personally... php files.......... NOTE * - this will pretty much be a membership records management website, but with some law enforcement needs added in..... thanks. The following is a best description as i can provide. **Membership PROFILES** Password Protected. Admin profile = that would be able to do everything the same, but have the added ability to create a profile for an officer. Corrections Profile = one that can do everything, add and edit records, but not be able to edit or add users. Officer Profile = Profile that can search, read information...nothing else. INSIDE Once logged in there should be a blank front page easily edited... there should be some form of navigation, nothing fancy, need very simple easy to skin code, text....pref on the left side as in a “navigation pane?? Options include. 1. Search Records 2. Add Records 3. Sex Offenders 4. Gang Taskforce (IF POSSIBLE ??" Number 3 and 4 should be “FILTERS?? on records. When adding a criminal profile the officer adding the profile should have a selection “type?? or similar, where they can choose wether or not this is a registered sex offender and or gang related incident. Drop down box should be easy enough to code. Admin profile should be able to add and delete the options that can be in the drop down box. And should be optional not required. When and iff the admin ADDS a filter it should be reflected on the front navigation pane. Such as if admin went into system, and added filter “idiot?? the navigation should re-appear such as 1. Search Records 2. Add Records 3. Sex Offenders 4. Gang Taskforce 5. Idiot. SEARCH RECORDS PAGE This should be very simple, basically a list box. Bringing up the database information. Obviously with the option to search. Search options should include. Last Name,First Name,DOB, drivers license number, state, city, address. The officer should have the ability to search the system with any or, not all required. The Listbox should have as follows when displaying results. Last Name, First Name, Middle Name, DOB, Sex, Address-state-zip, Status, Type,Last date edited, View Profile link “Type?? should show the filters if selected. Such as gang activity, or registered sex offender **Adding PROFILES** When adding a profile or editing a profile, the following fields are what’s needed in the database Lastname ??" txtbox Firstname - txtbox Middle ??" txtbox DOB ??" datebox (popup calendar) Sex ??" drop down Male female unkown Status ??" txtbox Type ??" dropdown (gang /offender so on as described above) Address ??" add ??" city ??" state ??" zip (should be separate so that each can be searched by on the search page) -- Booking photo A ??" uploading a second time over rights current photo for that profile Booking photo B ??" same as above -- PLEASE NOTE* - the arrest list and infraction list can actually just be big text boxes if it becomes too hard to code. As long as the text saves, and its infinity long,then good job. Should be simple. But if you can add a special menu for adding citations and stuff that would be awesome. Arrest List ??" this list should have three colums of information Arrest time/date = arrested by = agency Infraction list ??" this should be the list of infractions or the “citation?? given to the individual Statute ??" Offense ??" Class ??" Court ??" Required Bond ??" Amnt Paid Obviously a submit button to place into registry From here it’s simple. The FILTERS should just be list boxes such as seen on the search page, but without the search button. It should bring up all profiles registered under that filter in the listing. Same listing applies Last Name, First Name, Middle Name, DOB, Sex, Address-state-zip, Status, Type,Last date edited, View Profile link PROFILE VIEW In the profile the following should be structured.
