create a php form for vendor selection on existing site

Dear bidder, this is a modification on my existing website. below are the tasks that needs to be done. 1. Divert the home page to admin page. 2. Modify the drivers (vendors) form to add vehicle number, city. 3. Put filter city in administrator in sales and support types. Listing of driver should show only city-based drives. 4. Modify the order as small window in AJAX, bootstrap preferred. Link add-client function in same form. Here is the process I plan. When user calls, note down the phone and record it in system (client creation). After that further details like service type (drop down), from and to, pickup date and time. After the inputs show a window with 10 drivers the drivers with phone numbers. The drivers who has been assigned with an order will be placed after others. please refer to image to see the form style. 5. SMS will be sent to client once order is assigned.
