Create Pages on web-site to list Phone numbers from Calling Card companies

Attached is a sample excel file with access numbers, Access numbers are phone numbers of calling cards. The attached file is only a sample. The actual file has about 90,000 phone numbers. 2 excel sheets are phone numbers of the United States. One sheet are phone numbers of Global Access number. I need a page that will list All countries. Each country will be a link to a new page. On this 2nd page, if it's United States it will list a list of all states, and each state will be a link to a new page. This new page will list ALL phone numbers of this state. Each phone number will be a link to a new page, we will call this page the "Access Number Page". If it is not United States, it should list the phone number page when clicking on the country. And each phone number should be a link, the same as United States. I will need each page to be SEO friendly URL's. Example: [login to view URL], [login to view URL], [login to view URL] and the Access Number Page should be like this [login to view URL] This is for the web-site www.bhcards.com. We started doing something, but not finished, see here: [login to view URL] The "Access Number Page" should have in the page the phone number, plus a list of all countries and the countries will link to the county page on our web-site, example: [login to view URL] Also on this page, the phone number should be displayed as 1-718-534-2100. and as 718-534-2100 and as 7185342100 and as (718) 534 2100. and we will add some content to the page as well.
