Automated Proposal Form Wordpress Site

For a construction website an automated proposal form needs to be implemented into the website. The design has already been made (psd is available).
Based on the input of the customer the proposal will be sent to the user's mail in a .pdf file (quotation lay-out is is ready). Next a plugin needs to be built for back-office purposes with several subjects. The plugin must have a export function to excel (.xls).
