Demo App Needed

I've got an idea for a big database driven app. I need a quick demo for a proof of concept. The idea actually came about from a homework assignment I never did and there was only a week to do this so I don't think it's anything more than trivial for someone experienced with it. Successful completion may is most likely to lead to a 100+ page spec, sorry the attached is sloppy but hope you agree how simple it is. It just needs to look pretty and do this simple task of viewing the database records from a web server, be able to chance the database on the web server and see it automatically update on the app screen. See Word attachment for details. Please note that if you can't do in the environment requested, it can't work for me. Trying to prove to an investor how east the larger app can be private labeled and I need to make a copy of it in front of him, just change the text and pictures and show him. If works, will get $75KUSD and use you for a lot of parts of project.
