Custom Fields For HWD videoshare

*I need 15 custom fields to be created for HWD Videoshare component(joomla extension).

*All the fields are pretty much simple data fields like name, address, company, street, residence, etc.

*Basically they are all text & text area fields only (No Complicated Fields)

*I will provide the links via PM. I need this done in the next 2 days.

Clarification: I want those fields in the upload page & i want the date entered(i.e saved in database) to be shown in the video page(along with the other details, that hwdvideoshare already has).
