Online report from existing SRC MySQL database. A) create html report similar format to attached excel file ([login to view URL]) --try to format so prints all fields one wide page, x pages long B) manual input for fields identified by red color C) 'stat', 'src' fields are drop-down choices (see excel for examples) D) 'u_date', 'q-date' are date fields, default is date of report input, but user can change E) 'q_no' is up to 5 char numeric field, free-input F) 'q_amt' and 's_amt' are input $ figures, no decimal The remainder of the fields come 'up' on the input form so the user can see them when creating this report G) the report can be created or updated at any time... the manual input fields come 'up' so the user may change the values, or put values in if they are blank F) the header of the report is the date when the report is run H) the user accesses the report on an https page with a user ID and pwd. An excel file of the database is attached for reference, but reporting should be done from the MySQL version online (separate database table so as not to spoil the original database)...
