A Survey Site

Hi All, I need a simple web site that will display questions based on the previous answers in a Survey Questionnaire. The project would require you to write some basic content for the introduction page of the web site. I already have arranged what questions are to be [login to view URL] need to be organized into multiple pages. There will be 1 admin page where survey records can be searched and an option to export the searched results into an Excel file. Also, I would require tool tips where each complicated terms meaning would be described. The web site is specific to network marketing. The survey site should use a database to store all the inputs from the visitors. Visitors need to fill in a basic form having their mobile, email, location and other contact details along with their feed back before starting the survey. Again at the end of the survey, those details will be displayed in text boxes, asking them to confirm their contact details where the Survey results would be sent across. Results should be displayed in a % bar chart and not numbers. Results should be stored in a DB so that I can query the user details entered in the form, based on the answers provided. Thanks, Anil. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables): a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform The survey should be able to run on all browsers and all operating systems. I use hostmonster as my web site host. Hence, I would prefer to use MySQL as the DB backend. Also, Please try to increase the usage of words like Network Marketing, MultiLevel Marketing, MLM as much as possible. The word density I am expecting is around 5% for each of these words.
