Deep PHP & MySQL & Arrays knowledge needed: Create custom-form pages

You need to propose the database structure, and the admin and usage views, of a concept of "Action-Pages" such that custom quizes, surveys and similar can be built. Background: We have a database with contacts and a database with sets of web-pages (let's call them web-sets). Purpose of job: For each web-set, we need to be able to let the owner of the web-set to create a series of 1-n "action pages" which each will contain: 1) information from the admin to the contact, 2) questions from the admin to the contact with various types of input-fields, the answers of which needs to be stored for each contact together with the rest of the contact information 3) the possibility for the admin to set up counting rules (like +, -, /, *) to be able to calculate new variables from the data entered (by user or admin) 4) the possibility for the admin to write out an answer after fields are entered to be shown in the page using conditional statements (if-then-else etc) about the answers given in 2. above and earlier information stored in the contact database, and those references from the contact-database (including things like name, and answers given to questions as in 2. above) should also be possible to write out on the page after the answer. 5) create different actions that will happen at the end of one action-page depending on the setup of the admin and what has been entered by the user. The pages will be styled according to the location of a theme's own CSS. The implementation will HAVE to be done in PHP and MySQL, and Ajax/Jquery are encouraged if necessary.
