Write an iPhone application

I require an iPhone app to be created which will have two basic forms with text inputs. The app is for offline use and will save the records on the device, in addition to being able to save as a .csv file which can be added to an email message. I will supply the icons for the app in addition the colour scheme colours, however this is for internal use and not a public app so styling isnt the highest priority and can be basic in this regard. One form has 70 text inputs, mainly numbers and the second has 45 inputs mainly numbers. It would be nice for field validation (example integers only from 10,000 to 160,000), however the validation isnt a huge priority as data is validated when the .csv is used down the track. I will supply the field names, field type and validation criteria, in addition to an example output .csv file I anticipate a generic app template can be used, and for someone experienced with generating a .csv output from an iPhone app this shouldn't be more than a few hours work. Source code to delivered upon job completion, and my preference would be for it to be licensed under GPLv3 or similar.
