An Android app for inspection of beaches/harbors.

We need an Android app for inspection of beaches/harbors. It should be optimized for tablets.
The first page should be a login page. The next page should be a list over locations to inspect. It would be nice if there also could be a map with the locations.
When tapping on a location the inspection form should be presented. The form mostly consists of yes/no answers and a few text areas. In connection to the text areas it should be possible to insert pictures taken by the camera. There is one form for beaches and one for harbors.
When finished there should be a submit option and the data has to be saved or sent to the server.
It’s important that the app works offline. Of course the first log in should be done online.
