I have a four page pdf form created with adobe professional. I have already inserted most fields with adobe pro, but need help with javascript calculations Several fields require date calculations related to pregnancy and due date. I will provide calculations. The pdf will be viewed online (with reader only in most cases) and entered by clients and either e-mailed to me, printed, or saved.
1. One field would contain a last menstrual period (LMP) and another the cycle length (normally 28 days). Using those fields, the javascript will calculate the estimate due date.
LMP + (28-28) + 280 days = due date
If longer cycle, say 31 days; LMP + (31-28) + 280
If shorter cycle, say 25 days; LMP + (25-28) + 280
2. One field contains todays date and another contains the woman's due date. I would need a javascript to calculate the number of days she is into her pregnancy.
For instance today's date Oct 4 2008 and due date Jan 1 2009. Today she is 27 weeks and 2 days. This can be calculated by subtracting the days left in pregnancy, 280- (due date - todays date) = # days into pregnancy, and then convert to weeks and days.
3. Finally, using ultrasound dating, on a given date the woman is so many weeks and days pregnant, you would calculate the due date. Also, calculate BMI based on height and weight entered.
4. The due date would be used to calculate current weeks and days, when a new current date is entered. Part of the form is a visit summary, ie on this day (user enters date) and utilizing specified due date, the weeks and days would be calculated.
5. Finally, I'd like javascript if possible to highlight certain fields in the pdf based on the drop down field selected in another field. For instance is one field contain "yes", I'd like to automatically highlight another field in yellow. Not sure if this can be done.
6. Explain, and implement, if it's possible to have the patient complete the form online and submit the entire pdf using my own servers e-mail rather than relying on the individual to have an email program on her computer.
I'll provide all calcs, you simply need to know how to do the java script accurately. Should be a quick project for someone experienced. Has to be done within Adobe Pro.
