Convert my Forms to HTML with Javascript

Please read the requirements for these forms and how they need to be packaged to work with Forms2Go. [login to view URL] It is basiclly a standard sales form with itemized fields with totals, discount, tax, etc, account info, contact information, and numerous other related fields. It will also need to have some fields or pages hidden or displayed based on the input on the main sales quote page. On the main page I want an "Accept Quote" button that will then dispaly the additional pages and fields. I will attach the main form I tried to do that will show the main fields, (formatting is pretty good) but it does dont do anything. Only Freelancers with Good Ratings will be considered. All payment will be thru escrow only and not be released to any freelancer until a complete working form is delivered and accepted by us.
