We need a professional looking logo designed, this will be placed on a simple coming soon page that will include 3 fields (email, name, and location drop down list). Once user clicks notify me, the data entered by the user should be stored in a database, and an email to verify the email should be sent, after the user verifies the email, we should display another page where the user is told that their email was verified, and we will also display a questionnaire where the user will select (one or more) from a list and submit (we need this to be also stored in the database.
We also need a small 4 page website that will be static and 1 page that is a contact us form that needs to store data to the same database as above, we need an easy way to change the content and images. I need something that looks good, so we need to see good website design skills. We don't care if it is wordpress or HTML with Bootstrap or say PHP. We need the most cost effective and professional looking solution.
We need access to a good library of images, (free ones seem to have limited images), so can you also provide that?
The 4 pages will be similar to:
