Create a word template URGENT JOB - 1 DAY TURNAROUND

Hello Urgent job 1 day turnaround I would like to create a smart form compatible with word that can be saved as a template. Compatible with Ms Word for office 365. The main document to copy is ‘Zodiac Heat Pump Questionnaire 2016’. On this document I have amended things ([login to view URL]). I have included asterix to insert new items shown on the file ‘Heating Consultation additional items’. Footer also needs to be created, will send this to winner, plus my logo. Must be saved as a word template so I can use it over and over. Must be able to print well without any additional formatting. Must be able to convert to pdf. I would prefer that the form did not go over 1.5 pages. I expect there will be 2 drafts and reviews before we finalise. Thanks
