Build and replace a reponsive form on a template html file

We want a form replaced on a html file.
The new form must be four buttons, depending on which button is pressed a different form needs to slide down.
Maybe you can use expand and collapse functions.
Ie button1 press form1 slides down, the forms will also have to be multi pages and have validation.
The form should have the same questions as:

For the time being please leave it as 1 question at a time, if i wanted 2 questions on one step then you must show me how to do this.
It must use the same css file as already there, and if i change the style of the template that is selected in the template.
The results need to be emailed to us.
