Build online cleaning calculator

I would like someone to build a simple online calculator.
Basically, this would allow people to place data into fields, in which it calculates how much chemical, water and labour time people spend on their cleaning.
Here are two examples of calculators that are similar to what I require.

I already have a website that this will be built on.
I would also need the freelancer to set up an automatic email that is sent from us to the user with their results (table and pie chart form in a PDF). The user would need to fill out a form on the calculator with their details, so the automatic email with their results can be sent to them.
