I have 32 pages of scanned medical forms. These need to be typeset and cleaned up, and a logo needs to be added. See the attached sample for an example of desired output. Then supplied in Adobe InDesign (CS4 or earlier) format.

I am looking for a delivery by Tuesday, December 21 - this is very important. I am looking to spend less than $320.
