Create a PDF form with fields to be filled out with each field linking to an Excel Spreadsheet

I have a standard form that is filled out for each person that is interviewed (like a survey). Some are checkboxes, others are text. In a day, I might interview 100 people or more and only have my iPad or tablet with me. For each field that is filled out, I need the information to be transferred into a single EXCEL spreadsheet at the end of the day with each field representing a column in the Excel file. I need somebody to create this form (and make it look professional) and link to Excel. All project files must be confidential. All project files should be submitted at the end of the project.
