Database connection string in [login to view URL]

C# Window Forms Application

I have Database connection string in app.config. I want to create a form so that when u open it loads below in Text Boxes

1. Server Name

2. database

3. User ID

4. Password

5. Window Authentication as checkbox

Save button to update the credentials and this will validate the above information too
