Contact form with image upload

I WOULD PREFER TO OFFER THIS PROJECT TO AN EU BASED BIDDER AS I MAY NEED TO DISCUSS THIS ON SKYPE IN MY TIME ZONE. YOU MUST SPEAK AND UNDERSTAND GOOD UK ENGLISH. This will probably take a competent programmer less than two hours. I always buy on skill and quality, never on price. I have a website which is virtually complete except for one contact form that needs a couple of extra fields and a new second contact form with photo upload facility. 1. I will supply the field names and field specifications. 2. Site user must be able to upload up to three images. 3, The field data has to be redisplayed and offer the option to amend. 4. On confirmation click: a. Write the data to an Access database; b. Store the photographs; c. Send me a new data message onto my personal email address; d. Send the site user a confirmation email. The successful bidder will be given access to the beta site. You must use the same style and appearance as the ordinary contact form that is already on the site. I will choose the bidder by Tuesday evening. The work must be completed by Thursday evening. GAF escrow only. Please note I ignore bland "Let's start" bids. If you can't take the trouble to give a sample of your work then why should I choose you? I always ask for ONE, and bidders still think I want fifty. I don't. Please supply just ONE. I have neither the time nor inclination to plough through excessive information. If you can't understand this simple instruction, then I won't choose you.
